Medical

AVH offers medical coverage to all regular full-time employees and their dependent(s) effective the first of the month following their 90 day waiting period. AVH offers coverage through United Healthcare PPO Plan.

Coverage provided through our Preferred Provider Organization (PPO) uses the United Healthcare network. For in-network services, there is a $25 copay for a visit with a primary care physician, a $50 copay for a visit with a specialist, a $100 copay for a urgent care facility and a $150 copy for the emergency room.

Inpatient Services are covered at 80% after the deductible is met. The annual deducible is $2,000 per individual up to a maximum of $4,000 for the family.

If an employee and/or dependent choose to go out of the network, most expenses are covered at 60% with a deductible of $4,000 per individual up to a maximum of $8,000 for the family.

Each year, AVH will pay 50% of each Medical expense that is applied towards the deductible up to $1,000.

The Calendar Year Maximum Out-of-Pocket for In-Network Care is $4,000 per individual up to a maximum of $8,000 per family. For Out-of-Network Care, the maximum is $8,000 per individual and $16,000 per family (in addition to the deductible).

The Lifetime Maximum for this PPO plan is $5,000,000


Dental

AVH offers dental coverage to all regular full-time employees and their dependent(s) effective the first of the month following their 90 day waiting period.

The coverage is provided through our Preferred Provider Organization (PPO) using Humana network. In-Network preventive services are covered at 100%, while minor services are covered at 90% and major services are covered at 60% after the annual deductible of $25 for individuals up to a maximum of $75 for the family.

Out-of-Network preventive services are covered at 100%, while minor services are covered at 80% and major services are covered at 50%.

The Calendar Year Maximum for this plan is $2,500 per individual (excludes orthodontic care).


Vision

AVH offers a vision plan to all regular full-time employees and their dependent(s) effective the first of the month following their 90 day waiting period.

Employees who elect the United Healthcare Medical Plan or the Humana Dental Plan are automatically enrolled in their discount plan at no additional cost. For a list of the discounts, please refer to the summary sheet located in your Orientation Package.

Coverage provided through our Preferred Provider Organization (PPO) uses the Avesis network. Employees who elect the PPO option are eligible for one routine eye or contact lens exam per year by an In-Network provider with a $10 copay for the exam and a $10 copay for materials.

New lenses may be purchased every 12 months and are covered in full after copay. Contact lenses that are medically necessary are covered in full; if elective, there is an allowance of $110.

New frames may be purchased every 24 months. Other supplies and services are provided at no additional cost, other than the co-payment, unless optional items are selected that the plan does not cover. This may include, oversized lenses, tinted or photochromic lenses, coated lenses, or frames which exceeds the wholesale allowance.

Out-of-Network provider services are reimbursed from $25 to $250, depending on the service provided.

If you participate in our Medical plan, as a United Healthcare member, employees and their dependants may receive up to a 15% discount off the provider's usual fee for LASIK surgery through the "Laser Vision Network of America". Chiropractic Care

Employees who elect the PPO medical option are eligible for 20 visits to a participating Chiropractor with a $10.00 copay per visit.

Employees who elect the PPO medical option may visit a participating chiropractor; there is a $30.00 copay per visit. The Calendar Year Maximum for these services is $2,000 per individual. For spinal manipulation the Calendar Year Maximum is $500.00 per individual.


Flexible Spending Accounts

Employees may set aside up to $1,250 pre-tax dollars to cover approved health care expenses not covered by the Medical, Dental or Vision plans (co-pays, deductibles, coinsurance, etc...a list of eligible expenses are included in your Orientation Package.

AVH employees may set aside up to $5,000 ($2,500 if married filing separate tax returns) pre-tax dollars to cover approved dependent care expenses such as before and after school programs, nursery or pre-school tuition, summer day camp, care in a home by a licensed provider, or approved elder care.


Life Insurance/AD&D

AVH provides a $15,000 benefit of Life insurance and $15,000 benefit of Employee Accidental Death & Dismemberment (AD&D).


Retirement Planning

AVH offers a 401(K) Retirement Plan offering a variety of fund choices. Employees are eligible to enroll in the Plan the first of the month following their 90 day waiting period.


Holidays & Paid Time Off

AVH offers its employees six paid holidays and 10 days paid time off.


Employee Assistance Program

AVH offers its employees and dependents an Employee Assistance Program (EAP), though United Healthcare's program Care24. Care24 is a service that can provide information on a wide range of topics and issues including legal and financial issues, older adult and parenting resources, and personal and work-related problems. Strict confidentiality and privacy are assured to each employee and family member who uses this counseling service. This benefit is paid 100% by AVH.
